Questionnaires

A questionnaire collects information from the patient before the visit, and the answers feed directly into the consult note rather than sitting in a separate inbox for someone to transcribe.
Getting one set up
You do not build it yourself. CleverConsult will either:
- adapt the questionnaire you already use, so patients see something familiar, or
- build a new one from scratch for your clinic.
It is fully customisable — tailored to what your clinic actually needs answered, rather than a fixed set of questions.
Where the answers appear
On the patient's chart, the Questionnaires tab shows the responses. The content also flows into the consult note as one of its sources.
Controlling how answers are formatted
Responses are re-rendered into a doctor-facing format by a questionnaire output template — a separate template type from your scribe templates, under Templates → Questionnaire Output Templates.
One important behaviour: only your default template runs automatically when a response arrives. Additional templates exist, but they do not run on their own.
