Questionnaires
A questionnaire collects information from the patient before their appointment, and the answers feed directly into the consult note.
Getting one set up
CleverConsult builds the questionnaire for you. It will either adapt the one your clinic already uses, so that patients see something familiar, or write an entirely new one from scratch.
Either way it is fully customisable and tailored to what your clinic actually needs answered.
Where the answers appear
Responses show up on the patient's chart under the Questionnaires tab, and the content also flows into the consult note as one of the sources it is built from.
Controlling how the answers are formatted
Responses are re-rendered into a doctor-facing format by a questionnaire output template. These are a separate type of template from your scribe templates, and you will find them under Templates → Questionnaire Output Templates.
